NCLEX-RN practice questions with rationales

One sample from each of the eight client-needs categories on the 2026 NCSBN test plan, taken straight from the Aegis question bank. Answer, then open the rationale. Pick a category below for more.

Question 1 Management of Care Multiple choice hard

An RN charge nurse is making assignments. Four clients need care. Which client should be assigned to the LPN?

  1. A.A new admission with chest pain
  2. B.Stable post-op day 3 client requiring routine medications and dressing changes
  3. C.A client receiving blood transfusion that just started
  4. D.A client awaiting initial discharge teaching
Show answer and rationale

Correct answer: B · LPN assignment

LPN scope: stable predictable clients with established plans. Initial assessment, blood transfusion (RN-only initiation), and initial teaching require RN.

Question 2 Safety & Infection Control Multiple choice medium

A nurse is caring for a client with active TB. Which PPE should the nurse wear when entering the room?

  1. A.Surgical mask and gloves
  2. B.N95 respirator
  3. C.Sterile gown and gloves
  4. D.Standard precautions only
Show answer and rationale

Correct answer: B · TB airborne precautions

TB requires airborne precautions: N95 respirator (or PAPR), negative-pressure room, door closed.

Question 3 Health Promotion Multiple choice medium

A client uses a peak flow meter at home. A "yellow zone" reading (50-80% personal best) indicates:

  1. A.Asthma is well-controlled
  2. B.Caution — use rescue inhaler and follow action plan
  3. C.Emergency — go to ER immediately
  4. D.No action needed
Show answer and rationale

Correct answer: B · Peak flow zones

Green = good control (>80%). Yellow = caution (50-80%) — use rescue inhaler, may need oral steroid. Red (<50%) = emergency.

Question 4 Psychosocial Integrity Multiple choice hard

A nurse is caring for a sexual assault survivor. The most appropriate initial intervention is:

  1. A.Immediately bathe the client
  2. B.Provide a safe, private environment; preserve evidence; consult SANE (Sexual Assault Nurse Examiner); ask permission before any procedure
  3. C.Question the survivor in detail
  4. D.Inform family without consent
Show answer and rationale

Correct answer: B · Sexual assault care

Sexual assault: privacy, safety, trauma-informed care, evidence preservation (do NOT bathe before exam), SANE if available, consent for each step. Mandated reporting may apply for minors.

Question 5 Basic Care & Comfort Multiple choice medium

A client with right-sided weakness is learning to use a cane. The nurse should teach the client to:

  1. A.Hold the cane on the right (weak) side
  2. B.Hold the cane on the left (strong) side and advance it with the weak leg
  3. C.Hold the cane in front of the body
  4. D.Use two canes for stability
Show answer and rationale

Correct answer: B · Assistive device use

The cane is held on the strong side. Advance the cane and the weak leg together, then step with the strong leg. This widens the base of support.

Question 6 Pharmacology Multiple choice hard

A client with new-onset A-fib is started on apixaban. Which client teaching is most important?

  1. A.Take with food only
  2. B.Report any signs of bleeding (gums, urine, stool, bruising)
  3. C.INR will be checked weekly
  4. D.Avoid all leafy green vegetables
Show answer and rationale

Correct answer: B · DOAC anticoagulation

Apixaban (Eliquis) is a DOAC — no routine INR monitoring needed and no dietary restrictions. Bleeding precautions are essential. Vitamin K restriction is for warfarin.

Question 7 Risk Reduction Multiple choice hard

A client with chronic renal failure has BUN 78 and creatinine 4.5. Which is most likely a contributing cause?

  1. A.Diabetes and HTN (most common causes of CKD)
  2. B.Asthma
  3. C.Hypothyroidism
  4. D.Anemia
Show answer and rationale

Correct answer: A · CKD etiology

Top causes of CKD: diabetes (#1), hypertension (#2), glomerulonephritis. Manage these to slow progression.

Question 8 Physiological Adaptation Multiple choice hard

A client with heart failure has a BNP of 850 pg/mL. The nurse interprets this as:

  1. A.Normal value
  2. B.Indicates significant volume overload and ventricular stretch
  3. C.Indicates dehydration
  4. D.Indicates acute MI
Show answer and rationale

Correct answer: B · BNP interpretation

BNP > 400 pg/mL strongly suggests heart failure exacerbation. BNP rises with ventricular wall stretch from volume overload.

How to get the most out of practice questions

Read every rationale, even when you are right. The NCLEX tests whether you can tell the best answer from a merely acceptable one. The rationale is where that distinction lives.

Study by category, test mixed. Drill a weak category until it moves, then switch to mixed sets so you practice the context switching the real computer-adaptive exam demands.

Re-attempt what you miss. A question you got wrong three days ago and now get right is worth more than three new questions. Aegis schedules those re-attempts for you with spaced repetition.

Time yourself. You have up to five hours for 85 to 150 items, which works out to roughly two minutes per question. Practicing untimed trains a habit the exam will punish.

Practice question FAQ

How many practice questions should I do before the NCLEX?

Most candidates who pass report answering somewhere between 2,000 and 3,500 practice questions over 6 to 10 weeks. Volume matters less than review: read every rationale, including the ones you got right, and re-attempt what you missed a few days later.

What is a good percentage on NCLEX practice questions?

Consistently scoring 65% or higher on mixed-difficulty questions is a common benchmark that correlates with passing. Aegis turns that into a readiness score that also accounts for category coverage and your recent trend, so you are not guessing from a single percentage.

Are these the same questions as in the app?

Yes. These samples are pulled directly from the 1,093-question Aegis bank. In the app you get 25 free questions a day, timed sessions, spaced review of what you miss, and a readiness score.

How are select-all-that-apply questions scored on the real NCLEX?

Since the Next Generation NCLEX launched in April 2023, SATA items use partial credit scoring (+/- scoring): you earn a point for each correct option you select and lose a point for each incorrect one, with a floor of zero. You no longer have to get the entire item perfect to earn credit.
